Gathering Statistics

Although the WEX Summary Report is mandatory, how each region gathers statistical information is dependent upon their individual needs.

 

Log Entries

Caseload activity is recorded monthly on the WEX Log.  WEX Log Entries are as follows:

List participant’s name (last name first).

List participant’s case number below the dotted line.

New Referral

Enter a check mark in this column if the participant is assigned to WEX/CSR for the first time or is re-assigned after completing a previous WEX/CSR assignment.

New Participant

Enter a check mark in this column if the participant shows at the Work Site for the first time (Note: It is possible for participants to be referred to WEX/CSR on one month and show up [participate] at the Site in the following month).

ECM #

Record the Employment Case Manager’s Worker Number

Site #

Record the number of the Work Site to which the participant was assigned in the current month.

Start Date

Enter the date that the initial assignment is scheduled to begin.

Job Title

For new referrals only, indicate the title of the position the participant will have at the Work Site.

Hours Assigned

Enter the number of hour the participant was assigned to work in the report month.

Hours Worked

Enter the number of hours the participant actually worked in the report month.

Final Month

Enter a check mark if this is the final month of the assignment.

 

Log Totals

On the bottom of each log page, the ECM shall total the number of:

 

·  Participants

·  New Referrals

·  New Participants

·  Hours Assigned and

·  Hours Worked

 

Note: In order to obtain the number of “hours worked”, ECMs may have to call the Work Sites that have not returned the Work Experience Attendance Report (27-302 HHSA) by the 5th day of the month following the assignment month. 

 

Case Count

The ECM shall report the total number of WEX/CSR cases and the number of new referrals monthly on the Caseload Report form. 

 

The Caseload Report is due to the:

·  ECM Supervisor by the 6th calendar day of the month following the report month and

·  WEX Administrator by the 8th calendar day. 

 

WEX Log Summary

A WEX Summary Report is due to the:

·  ECM Supervisor by the 5th calendar day of the month following the report month and

·  WEX Administrator by the 13th calendar day. 

 

WEX Administration will prepare a report for WTW by the 15th of the month following the report month. 

 

Completing the WEX Log Summary

The WEX Log Summary is completed as follows:

Enter the total number of new referrals scheduled to begin their assignment in the current month.

Enter the total number of new participants (persons who showed at the Work Site for the first time).
